Training and clinical background

Dr Ghosh completed MBBS and then MD in General Medicine at the Institute of Postgraduate Medical Education & Research, Kolkata — a three-year postgraduate residency in adult internal medicine covering cardiology, respiratory medicine, gastroenterology, endocrinology, nephrology, neurology, infectious disease and critical care as they present to a general physician.

The practical significance of that training is the sequence it imposes. History and examination come first; investigations are chosen to answer a specific question rather than ordered as a panel; treatment follows the diagnosis rather than the symptom; and a review date is set. Where the diagnosis is not apparent, the method is to construct and work through a differential systematically. Most of what a consultant physician does that a shorter consultation cannot is contained in that discipline.

Teaching

Alongside clinical practice, Dr Ghosh is Assistant Professor in the Department of General Medicine at MJN Medical College & Hospital, Cooch Behar, the district's medical college, where he teaches undergraduate and postgraduate students and supervises clinical training on the wards. Teaching and practice reinforce one another: preparing to explain a decision to a resident tends to expose whether the reasoning behind it is sound.

Point-of-care ultrasound

Dr Ghosh holds the Point-of-Care Ultrasound Fundamentals Certificate from the Alliance for Physician Certification & Advancement (APCA / Inteleos), awarded in January 2019. Bedside ultrasound is not available at Uttarbanga Clinic at present — the training is in place, the equipment is not, and it is planned for 2027 alongside a dedicated abdominal sonography service.
